Dear Client, Fall 2006

Thank you for your interest in our client computer program, “Project Connect”.  Through this program local companies donate used monitors, cables, mice/trackballs, keyboards and CPU’s (central processing units, often called towers) to MSSP so that those diagnosed with MS in Oregon and SW Washington can remain connected with the outside world.  Next, MSSP volunteers carefully inspect each machine, download operating software, install modems and then deliver and provide limited on-site training in the privacy of the MS person’s home!  The cost for the machine and training is free thanks to the generosity of private contributions and grant support.  Unfortunately due to budget restrictions we cannot provide voice-activated software or printers.

As you might imagine we’ve had an overwhelming response from the MS community for computers.  We are eager to get computers out to each and every person.  To get started, please complete steps 1, 2, 3, and 4, and return all required documents to MSSP by the appropriate date for this season.

Step 1  Provide MSSP with a letter from your doctor that verifies your MS
Step 2  Click Here to print out and Complete the Computer Survey
Step 3  Click Here to print out, read and sign the Client Agreement
Step 4  Mail all documents to MSSP at: P.O. Box 16553 Portland, OR 97292-0553

Program Deadlines (Date Steps 1-4 listed above must be received by MSSP Office)
            Fall/Winter 2007    ……….………………December 31, 2007
            Spring/Summer 2007 ……………………..June 30, 2007

If you have any questions don’t hesitate to call the office at (503) 297-9544.  Again, thank you for your interest in “Project Connect”.

Sincerely,
sig
